I authorize Farmstead Veterinary Services to examine, treat, and provide veterinary care for my animals. I understand that payment is due at the time services are provided, unless other arrangements are made in advance. For some procedures, such as surgeries, farm calls, or herd work, a deposit may be required.

If an account balance remains unpaid, a monthly service fee of 1.5% may apply. I also understand that if my account is sent to collections, I will be responsible for any additional fees.
